Upon joining, each Scout is required to have a BSA informed consent/ health history form filled out and kept on file with the troop before participating in any troop activities – including meetings. The newest forms include both the consent and the medical history on one document (Parts A & B). A parent must update these forms annually.

Scouts (and adults) are required to have a Part C medical form for outings lasting 72+ hours – and this must be signed by a doctor after a physical exam. Tip: you can usually obtain this at your annual check-up or by visiting a med clinic. Please let us know if your Scout has any medical history, allergies, or takes medication on a regular basis. All Adults must also have a medical form on file with the troop to participate in any troop activity.
